Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of an RN or physician. The LPN supports patient treatment plans by administering medications, monitoring patient conditions, performing basic nursing procedures, and assisting patients with daily living activities.

This role plays a vital part in ensuring patient comfort, continuity of care, and effective communication between patients and the healthcare team.

Job Responsibilities

Patient Care & Clinical Support

  • Provide direct nursing care under the supervision of an RN or physician

  • Monitor and record patient vital signs, symptoms, and medical histories

  • Administer medications, injections, and treatments as prescribed

  • Assist patients with activities of daily living (ADLs) including bathing, dressing, feeding, and mobility

Documentation & Compliance

  • Accurately document patient assessments, care provided, and responses to treatment

  • Maintain patient confidentiality and comply with HIPAA regulations

  • Ensure adherence to facility policies, procedures, and state nursing regulations

Patient & Family Communication

  • Educate patients and families on care plans, medications, and health management

  • Provide emotional support and respond to patient concerns compassionately

  • Communicate changes in patient condition to RNs and healthcare providers promptly

Infection Control & Safety

  • Follow infection control protocols and safety standards

  • Assist with wound care, catheter care, and specimen collection

  • Maintain clean, safe, and organized patient care environments

Team Collaboration

  • Collaborate with RNs, physicians, therapists, and other healthcare professionals

  • Participate in care planning and interdisciplinary team meetings

  • Support quality improvement initiatives and patient safety goals

Skills + Experience

Skills & Qualifications

  • Active LPN license

  • Knowledge of nursing fundamentals and patient care techniques

  • Strong attention to detail and reliability

  • CPR/BLS certification
